Insurance

As part of your new patient paperwork, you will receive an Insurance Coverage Verification Form through your Charm online portal. It is your responsibility to call your insurance company to verify the specific benefits for which you are covered. The Insurance Verification Form will guide you on what to ask about, such as in-network vs out-of-network coverage, copay, coinsurance, lab and imaging coverage, and your deductible.

As this is part of your new patient paperwork, insurance verification must be completed within one week after scheduling your appointment. If this has not been completed within this week, your appointment will be moved to the next available time.

LABS & Tests

Amenda Clinic works with specialty labs that offer deeply discounted cash labs for patients with no insurance. We will always work with you to get quotes and decide what you can afford before you get the service.
