the new patient process

Step 1 - Submit an Inquiry Form

The first step to working with our clinic is to submit an inquiry form. This will take 5-10 minutes to complete and will let us know if you are looking for functional mental health or counseling. Our new patient coordinator will review your form and contact you. Our new patient contact days are Mondays and Tuesdays.


Step 2 - Intake Call with Coordinator

During your intake call, Amenda’s new patient coordinator will review your inquiry form

We want to know what kind of care and support you have received in the past and what you are looking for now. Are you looking for a mental health diagnosis or do you already have one? Are you looking for functional and naturopathic medical support or for counseling? Are you looking for medication management? Are you looking for integrative mental health care? We want to be sure we are the right fit for you and understand how we can best support you!

Part of this call will be to collect information that will be useful for your first appointment. We recommend being prepared with the location, phone number, and fax number for the following:

  • Your primary care provider

  • Your prescriber, if different than above

  • Your therapist/counselor

  • Recent lab work (within the last year)


Step 3 - Schedule an Appointment

During your intake call, you will be provided with a recommendation for one of Amenda Clinic’s providers. Now it’s time to get you scheduled! The soonest available appointment is typically 3-4 weeks after your intake call for all providers except for Dr. Vera Alcorn, whose soonest available appointment is typically within 3-4 months. Please note that Lauren Loos, LPC has limited availability.

As part of scheduling, an account will be set up for you in the Charm online portal.


Step 4 - Fill Out New Patient Paperwork

Paperwork will be sent to you through your new Charm online portal. This will include the following forms:

  • Informed Consent for Treatment

  • Pre-appointment Questionnaires

  • Health Intake (not required for those seeking counseling only)

  • Insurance Coverage Verification

Your new patient paperwork must be completed within one week after scheduling your appointment. If your paperwork is not ready to go at this time, your appointment will be moved to the next available time.
